66-year-old female presenting with bilateral ruptured silicone implants. She underwent breast augmentation approximately 40 years prior via an inframammary subglandular approach. She developed capsule contracture as well as distortion of breast shape. She underwent explantation and total capsulectomy via an extended inframammary approach. She was replaced with smooth round silicone implants in the sub muscular position. She is shown four months after surgery with improved contour and much softer breasts. She is very pleased with her results.
