52-year-old woman underwent bilateral total skin sparing mastectomy for breast cancer. The mastectomies were performed via a “Wise pattern” skin excision similar to a breast reduction. Immediate reconstruction was performed with tissue expanders and acellular dermal matrix (Alloderm). Second stage reconstruction was performed with placement of a permanent silicone implant. This patient is shown 5 months after second stage reconstruction.

This is a breast reconstruction case using tissue expanders, aceullular dermal matrix, and permanent implants or TRAM Flaps. The mastectomy was performed via a “Wise Pattern” Skin excision similar to a breast reduction. This approach is used in woman with larger breasts to reduce the amount of excess skin, giving a much improved shape and contour.
