50 year old underwent bilateral total skin and nipple sparing mastectomies with immediate tissue expander reconstruction. A single inframammary incision was used. Second stage reconstruction was performed 4 months after her original surgery and consisted of tissue expander implant exchange and fat transfer. She is shown 4 months after second stage surgery and is pleased with her results and is well.
